Male recovery home with a phase 2 sober living house

Have an online registration) at kbrh.org. Fill out the application and the clients must then call every Monday between 7 and 8AM, until they have an opening. They are a 14-18 beds in house residential recovery home, based on the Icelandic model. They have programming 2X a day Mon-Fri, with one 1-on-1 counselling session a week, as well as in person community AA/NA meetings, job and housing planning, medical access. Beyond that the residents have lots of freedom to come and go. Cell phones are allowed, as are laptops, wifi is free. If you are caught using, they send you to a 4 day hospital detox in Sault Saint Marie, then you return. If it happens more than once, the client is evaluated on a person to person basis. Men only. It is a 90 day program, with a Sober Living Home available afterwards for up to 18 months. Alumni are welcome to attend any in house program sessions. It is generally free, but they may ask you to contribute to rent if you are able. Residents also help maintain general chores to keep up to home. Food is excellent. Residents prepare uncooked breakfast themselves, and have a lunch and dinner time.

Hope Haven exists as a program to help those suffering from homelessness by giving them a safe, clean environment that also includes access to two case workers, as well as the support of the local Salvation Army Ministry and our local Social Assistance Administration Board. While at Hope Haven, residents have the opportunity to physically and mentally stabilize, access multiple different community supports, while also gathering insight for the next step in their journey - A Way Forward.

When you call or text 9-8-8, you will reach a responder at one of nearly 40 local, provincial and territorial, and national crisis lines, who co-deliver the service.

9-8-8 Partners are the backbone of the service. They are located across Canada. They provide existing crisis services within their communities, as well as taking 9-8-8 calls and texts. They are here to make sure that you can connect to a responder within your local community. Other 9-8-8 partners include Hope for Wellness who offer specialized support for First Nations, Inuit and Métis, and Kids Help Phone, who specialize in supporting young people reaching out to 9-8-8.

• North Bay Regional Health Centre (Our Schedule 1 Facility)
  • Adults 18+: The Acute Inpatient Psychiatry Unit (AIPU) unit provides high-intensity, acute psychiatric services 24 hours a day for individuals experiencing acute psychiatric symptoms and/or at risk of harm to self or others.  The referral must be Physician led to the On-Call Psychiatrist  (705) 474-8600 ext.7200.  The patient must be medically cleared prior to acceptance and transfer to NBRHC.  Consultations are available anytime during the assessment, treatment and management of patients at the Temiskaming Hospital.  **If there isn't a bed available, it is the responsibility of NBRHC to contact other hospitals for bed allocation.
  • Pediatric (less than 18yrs):  Physician to contact the Pediatric Unit (705) 474-8600 ext 4250 or Pediatrician on-call after hours (705) 474-8600 ext 7200. The admitting doctor notifies Bed Allocation which informs unit lead of admission.  Consultations are available anytime during the assessment, treatment and management of patients at the Temiskaming Hospital.  **If there isn't a bed available, it is the responsibility of NBRHC to contact other hospitals for bed allocation.

Temiskaming Shores Area

• Central Booking (x2289): The Temiskaming Hospital has access to visiting psychiatry for both Adults and Youth through Central Booking for outpatients.  Psychiatry typically sees the patient for assessment only.  There is a waiting period of approximately 3-6months.  Physicians can refer directly through dictation, and are encouraged to complete the Temiskaming Hospital Psychiatric Referral Form.  Health care partners such as Social Workers/Occupational Therapist/etc.. whom the client follows clinically, can refer patients by completing the Temiskaming Hospital Psychiatric Referral Form, attaching a biopsychosocial assessment providing pertinent information to referral needs, and providing the level of priority.
• Canadian Mental Health Association CMHA:  Community visiting psychiatry at CMHA is available when:
  • The individual is an active CMHA client receiving counselling services and the clinician faciliates the psychiatric referral.    There is a waiting period of approximately 6months to access psychiatry once the patient has been assigned to a mental health clinician.
  • Primary care refers to community psychiatry and are able to attend the psychiatric appointment to facilitate the psychiatric referral.
• SickKids TeleLink Mental Health Program: TeleLink provides expertise in the area of child and adolescent mental health to the province of Ontario and beyond. Through the use of live video broadcasting we connect children and youth, their case managers and families to psychiatrists and other mental health professionals for clinical assessments, capacity building and education.  Referral forms can be found on their website: https://www.sickkids.ca/tele-link/index.html The waiting period is approximatly 4months, however there is possibility of being seen more urgently if noted.  Fax referral to: (416) 813-8351
• Seniors Mental Health Program CMHA: Referrals must be made through primary care and is not an emergency service.  Referrals must been the criteria of: 65+, mental health problems complicated by age-related medical illness/functional needs, must be medically stable, and must reside in the community.  This program is a consultation service and consists of two Geriatric Psychiatrists.  

Regional Specialized Mental Health Programs:

• NBRHC Evergreen Lodge 65+:  In-Patient Seniors Mental Health Unit for late onset (age related) psychiatric disorder.  Referral can be found online.  Fax the referral to North Bay Regional Health Centre Inpatient Mental Health Units Regional Mental Health Central Referral (705) 495-7843.  Criteria: Must exhaust local psychiatric/geriatric services (including acute psychiatry in North Bay).  The patient must be assessed by a psychiatrist and trialed treatment options prior to referral.  Also have connected with the BSO Behavior Support Ontario, Seniors Specialized Program (705) 675-9193 x8903.  .
• NBRHC Birch Maple Lodge Dual Diagnosis Inpatient Unit: Birch Maple Lodge provides short-term (up to 18 months) inpatient assessment, treatment and rehabilitation for adults (18+) with a moderate to profound developmental disability and mental health concerns (dual diagnosis). 
• NBRHC Oak Lodge Regional Dementia Care Program: The Regional Dementia Care Program provides comprehensive specialized assessment and treatment of older adults (usually >65 years old) and/or adults with an age-related dementia, which is complicated by behavioural and psychological symptoms, and/or medical comorbidities that exceed the capacity of the community, general hospital, complex continuing care and long-term care system.
• NBRHC Nickel Lodge Specialized Adult Rehabilitation and Transitional Service: Specialized Adult Rehabilitation and Transitional Service which provides care and support to those with a severe mental illness who often present with a co-morbid condition. It is a specialized unit that is patient driven, recovery oriented and integrates patient wishes and goals through a wellness framework. The goal is to successfully address the problems that have limited successful transition into the community by maximizing individual potential, reducing mental health and behavioral challenges, teaching new skills to optimize functioning, improving relationships and enhancing quality of life necessary to return the patient to the community, whether that is to their home, or a supervised living arrangement.  In order to access services must complete the Regional Mental Health Central Referral Form. 
• NBRHC Northern Lights Lodge Intensive Rehabilitation:  Northern Lights Lodge provides tertiary mental health services to individuals within the North East region who are experiencing serious mental illness requiring a longer and more intensive approach to assessment, treatment, and rehabilitation than is provided by district level inpatient services.  In order to access services must complete the Regional Mental Health Central Referral Form.
• NBRHC Osprey Lodge Intervention & Self Care: Osprey Lodge provides care and support to those individuals experiencing a serious mental illness with a focus on maintaining or increasing their ability to care for themselves, wellness planning, skill building, placement, and follow up. A person directed approach allows patients to regain control of their lives as partners in an interprofessional team. Interventions aim to increase wellness and autonomy, to develop essential life skills, and supports to ensure a successful discharge.  In order to access services must complete the Regional Mental Health Central Referral Form.

Closed: Session exclusively for those who suffer from substance abuse and who have desire to stop.

Open: Session open to all interested members of the public, including members and/or their families.

• Referrals welcome. Individuals referred are contacted within a two week period by the intake worker to schedule an intake and to come in to sign consent forms for release of information. Clients who do not have a psychiatric diagnosis or with no history of psychiatric difficulties are encouraged to come in on a crisis intervention basis or brief service. If there is a waiting list, clients are contacted on a weekly to bi-weekly basis for a follow up and encouraged to utilize crisis intervention if in need.

Primary Care

Senior’s Mental Health Program 

• This program is a consultation service that is offered in the Timiskaming area.  The Seniors' Mental Health team consists of two Geriatric Psychiatrists and nurse clinicians who are committed to meet the needs of the elderly.  Following an in-depth assessment, a consultation report with recommendations as well as a follow-up plan is forwarded to your family physician.  Your care will continue to be coordinated by your family physician in order to maintain a high quality of life in the least restrictive environment.
• The referrals to the program MUST be made through your family physician or your nurse practitioner.  This is not an emergency service.
• Client Criteria:
  • Seniors (65yrs or older) with complex mental health needs, or younger than 65 who have a confirmed diagnosis of Alzheimer's disease or frontotemporal dementia.
  • Mental Health Problems: complicated by age-related medical illness/functional needs.
  • Must be medically stable; not in medical crisis
  • Must reside in the community: living at home, supportive housing, or in a facility based environment such as long-term or chronic care.

Supports residential programs, family homes and independent living homes. Certain locations host day programs where individuals can take part in leisure activities and skill building. Also helps with integration into the community in terms of employment and community involvement.

DDS works with patients, families, family doctors, and community based teams in the patient’s community and home. Care is provided with a patient and family centred approach and considers physical, mental health needs and any problems in their home or life.

The core team in North Bay works with community based Mental Health Clinicians located in key districts throughout the region.

Referral Process: 

Referral sources are required to complete a referral package prior to all consultations, including a written request for consultation from the patient’s general practitioner or primary health care provider. Call the DDS Mental Health Clinician at the agency nearest you.

Our criteria for consultations are as follows:

• Individual is 16 years of age or older and,
• Is being referred for a diagnostic assessment or review of medication/treatment plans and,
• Has a recognized developmental disability of a mild to profound severity and exhibits behaviours that may be attributable to a mental illness, or*
• *May have a diagnosis within the autistic spectrum disorders and is not adequately supported by local generic mental health services.*
• Individuals will not be assessed without their consent or consent from their substitute decision maker, and
• If there is a primary physician/psychiatrist, he/she must provide a written referral.

Call to arrange for an assessment, they work to create a support plan and connect you to the best services for your needs.

For eligibility, you will need to provide documents that show:

• you have a developmental disability,
• you are of age 18 or older,
• you live in Ontario.

There is also a Home Repair Funding Program.

Eligibility for Social Housing: 

• At least one member of the household is 16 years of age or older and is able to live independently with or without support services;
• Each member of the household is a Canadian citizen, or a landed immigrant, or a refugee claimant; No deportation, departure, or exclusion order has been made against any member of the household;
• No member of the household owes rental arrears to any housing provider under any housing program;
• No member of the household has been convicted or any offence under section 85 of the SHRA, HSA or crime under the Criminal Code of Canada regarding rent-geared-to-income assistance, or found to by a court of law to have misrepresented their income for the purpose of rent-geared-to-income

Eligibility for Home Repair Funding: Households must:

• Own a home that is their sole and principle residence with a market value at or below the average MLS resale price for the Timiskaming area and have an
• annual gross household income at or below $63,300.

As the intake and assessment unit, it provides comprehensive specialized assessment and treatment for older adults with complex age related psychiatric needs. Older adults exhibiting any of a broad range of affective, cognitive, or behavioural symptoms (usually representing a significant negative change from their previous levels of functioning) may be considered for admission to this tertiary care regional service.

For eligibility: You must provide 1 piece of identification for each individual living in the household, and household income and expenses

28 day program (with possible extensions as needed). It is mostly self-referral, but agency, or peer referrals can help. Daily programing includes meditation,  physical health work, CBT/DBT, healing circles based around PTSD healing and, dealing with trauma, aboriginal teachings, a once a week one on one with a personal counsellor. Food is very good. Free Funding. Residents are asked to do basic cleaning and upkeep. Three in person meetings a week, with access to Zoom. Clients can then register to a longer term treatment centre if they need it. They have a dedicated doctor and medical team, and deal with methadone, suboxone, and sublocade, as well as physical and mental health issues. Med are dispensed by staff 3X per day. Residents attend a store for snacks, and cigarettes (allowed) 2X a week. They will not release a resident with no place to go.

They also have a safe bed side, for emergency detox, homeless coupled wih SUDS, and justice beds available. A nurse is on staff there 24/7. And yu basically sleep, eat, watch TV and try to feel better, with the goal of transferring over to the other side . General stays here are 2 weeks, but can be 6 weeks or even longer. They are CO-ED, with around 12 in house treatment beds. They have counsellors trained in dealing with dual diagnosis.

How to Access Program:

If you are a patient at an Ontario Family Health Team (FHT), Community Health Centre (CHC), or Addictions Agency, contact your health care provider to enroll in the program and receive ongoing smoking cessation treatment, including Nicotine Replacement Therapy and counselling support, at no cost.

The programs includes:

• Community Addiction Treatment Service (Outpatient)
• Residential Addiction Treatment Services (including 21-day program) (Inpatient)
• Withdrawal Management – Detox (Inpatient)
• Concurrent Disorders (Outpatient) 
• Methadone Case Management (Outpatient)

Their Safe Beds and Withdrawal Management Services are walk-in 24/7, no referral necessary. They can do completion of ADAT by appointment.

Acute Inpatient Psychiatry Unit AIPU (For Adults 18+)

The Acute Inpatient Psychiatry Unit (AIPU) unit provides high-intensity, acute psychiatric services 24 hours a day for individuals in acute psychiatric distress and who are 18 years of age and older experiencing acute psychiatric symptoms and/or at risk of harm to self or others.  

Does not accept patients who are: acutely medically ill; acutely intoxicated; or who have a diagnosis of dementia and/or delirium.

Contact by dialing NBRHC main switchboard and asking for the on-call psychiatrist.

The referral must be Physician to Psychiatrist  contact and all patients must be medically cleared prior to acceptance and transfer to NBRHC.  Patients can be admitted to the AIPU through the on-call psychiatrist (705) 474-8600 ext.7200.  The admitting doctor notifies Bed Allocation which informs unit lead of admission.   Consultations are available anytime during the assessment, treatment and management of patients at the Temiskaming Hospital

 **If there isn't a bed available, it is the responsibility of NBRHC to contact other hospitals for bed allocation.

Child and Adolescent Mental Health Unit:

Both crisis and planned services for children and adolescents under the age of 18 with serious mental health problems. Crisis services are usually acessed through hospital emergency services. Referrals for planned non-crisis assessment and treatment require scheduling a preadmission session which engages the child/adolescent and family as well as referral sources in informed discussion.

The Nipissing and area crisis line is available twenty-four hours per day, seven days per week to provide mental health assessments, support and referrals.

Offers behavioural assessment & treatment, education, support and links to specialty care for:

How to Apply:

• Health service providers may enroll in the electronic database by filling and forwarding the Request Form (see below), or by calling their toll free number. This is to setup an account on the electronic intake website.
• Anyone, inlcuding family, caregivers or health service providers may refer a patient using the electronic intake form. 

There are also a Community Outreach Service that provides support to patient, family and community resources during the transition to community living after discharge. Education and skill development provided to patients, families and community. Integration and development of the necessary community resources, to ensure successful community living while optimizing individual strengths of the patient.

Admission Criteria for the Hospice Bed

• have a progressive life limiting illness;

• have a Palliative Performance Score (PPS) of 40% or less; priority will be given to a lower PPS scoring;

• live in the district of Timiskaming and surrounding area, or wish to return to this area to be with familyat end of life;

• no longer wish to receive active disease modifying treatment;

• “Do Not Resuscitate (DNR)” has been established through the physician’s order, and DNRC form;

• have care needs that exceed the supports available from the family (primarycaregivers) &/or care needs that exceed the supports available in the community or the individual does not wish to die at home;

• have consented to admission to hospital / hospice care and will be accompanied by family members as required;

• have a life expectancy which is anticipated to be less than 3 months; and

• have been assessed by a Physician or Nurse Practitioner (N.P.) within the last two (2) weeks.

First Link: is a referral program designed to help newly diagnosed people with dementia get the help they need as soon as possible. The Alzheimer Society provides services and support at the time of diagnosis and throughout the duration of the disease. Individuals and their families are linked to learning, services and care as early as possible in the disease process.

How to apply:  Anyone diagnosed with alzheimers, concerned friends, family or caregiver can self-refer to First Link. They can arrange for an appointment to assess their individual needs and be connected to the ideal supports.  Call any of the number below, or visit an office.

The goal is to assist patients in asserting themselves in their care planning to enhance quality of life and wellness, decrease behavioural challenges, teach new skills, and improve relationships.

Co-Dependency Program:

Residential program for males and females age 18 years and older who are affected by another person who has a substance abuse issue (either ongoing or in recovery), but who do not have one themselves.  It is to be noted this program is offered to individual family members, rather than whole family units.

Two Chemical Dependency Programs are offered:

Young Adults Group: For males only, ages 17 to 24. Clients attend sessions with the adult community, but group and educational sessions are geared toward the specific issues facing young men.

Adults Group: For males and females,

Clients attend lectures, videos, discussions, group counselling and 12 Step meetings. Issues and problems are addressed in light of personal experience and feedback.

The process of recovery is initiated and nurtured in a warm and supportive atmosphere. These programs are 3 weeks of residential instruction preceded by a week of pre-treatment with Withdrawal Management Services (WMS). An additional week of formal WMS treatment is also available.

Stage 2 Recovery Program: 12- Day residential program for men and women ages 18 years and up who:

• Have previously completed a residential drug and alcohol program 
• Are struggling to maintain a contented lifestyle
• Have had a minimum of 2 months clean and sober (consecutively)
• Are able to arrive for treatment clean and sober, 1 month off all substances (alcohol, drugs and non-approved prescription medications)
• Have been able to achieve lengthy sobriety through 12 Step involvement, counseling or own efforts, but who are requiring additional knowledge and assistance with enhancing their recovery.

Treatment Topics Include: Triggers to Relapse, Identifying High Risk Situations, Unhealthy Behaviors, Coping Mechanisms, Anger Management In Sobriety, Use of Leisure Time in Sobriety, Understanding the Relapse Process.

Applicant's admission information will be assessed by a team to ensure the participant is physically, emotionally and psychologically able to participate actively in all phases of the program. They provide a transportation program  (Wheels of Hope) for those who need to get to treatment. There is a registration fee for this program, if you are unable to pay the fee you may be eligible for assistance through their Compassionate program. Call Wheels of Hope for either of these programs.

Primary focus on students aged 17 to 25. However, anyone who does not fit into that group will receive referrals to other services based on an assessment of their needs.

Foodbank runs every Tuesday by appointment only. We ask that clients call no later than Monday to book their food bank appointment. Please call 705-647-3743 to book your appointment. 

The Food Bank is located in our church at 260 Whitewood Avenue in New Liskeard Ontario.

Upon completion of taper, clients participate in a 5 day pre treatment program, followed by a 4 week residential program. Clients who are using Benzodiazepine medications will need to be off this medication a minimum of 2 weeks prior to being admitted. Typically the wait time to get into the program is between 3 to 4 months.

The client must provide:

• $250.00 certified cheque, money order or bank draft made payable to The Oaks Centre to cover the cost of *Suboxone Treatment*. 

This payment is non-refundable and is required prior to an admission date being given.

Upon admission, a valid drug benefits card as well as $250.00 additional cash to cover the cost of other medications that may be prescribed to ease withdrawal symptoms.

Complete and fax assessment and once received, it will be reviewed by staff, and they will contact you at the contact number provided. An initial interview will be done over the phone, or in person, to assess your needs and gather more information and complete the intake process.

Key Points:

Mood altering medications are accepted on a case by case basis, and can be discussed with staff during the initial intake process.

Any residents on methadone/suboxone, must be on a stable dose. Breton House can discuss further information regarding methadone/suboxone during the initial intake process.

Requires a minimum commitment for three months and can stay for up to one year.

Assigned a primary counsellor also be put on a buddy system for two weeks upon admission.

Residents are expected to participate in 12 Step meetings, groups at Breton House, and groups in the community. Residents are assigned therapeutic duties, and are expected to complete them as required.

Referrals:

Applicant must call to initiate intake process.  A counsellor will get back to the client to set up either a phone intake appointment for those out-of-town, or a face-to-face intake for those in the Sault Ste. Marie area. This appointment is to complete the intake procedure. It can take from 30 to 60 minutes to complete.

All applicants to the program must be assessed using standardized assessment screening tools (ex. ADAT). Clients referred from out-of-town are asked to arrange for an assessment in their own community to be forwarded to Breton House.

Interview:

When the Ontario Telemedicine Network (OTN) is available, they will interview clients using this option. All women admitted to thr programs must have addiction specific issues in order to meet their admission criteria. They do accept clients who have co-occurring mental health issues but addiction must be the primary issue. Breton House offers both pre and post treatment services.

Admission to their community based counselling service (out-patient) is limited and available to local women only.

FAQ: Do I have to attend a hearing?

If your claim can be assessed based on written evidence alone, you won't need to attend a hearing. One of our adjudicators will make a decision based on the information in the file. This is called a documentary hearing. If an oral hearing is needed, you must be present. At an electronic hearing you will be on the phone or video conference while the adjudicators are at a hearing site. During an oral or electronic hearing, the adjudicators will ask questions about the incident, injuries and associated costs.

Application Process: Please contact the Children's Services Case Aide to start the screening process (new client), or make an appointment (existing client).

Once screening is complete, the Case Aide will provide a list of information the client will need to provide to the Child Care Eligibility Worker to proceed with the application. Both new and existing clients are reminded that when attending their interview with the Child Care Eligibility Worker, they need to bring the necessary documentation to establish eligibility.

Documentation can include:

• Verification of income using the federal government Notice of Assessment or Canada Child Tax Benefit for the most recent tax year;
• Verification of employment (2 current pay stubs) or education;
• All other relevant information as requested by the Child Care Eligibility Worker.

The application for Child Care Fee Subsidy will consist of an interview with the parent(s) and completion of the following:

Well Women's Clinic
The clinic specializes in the provision of performing breast exams, teaching breast self-examination, and performing pap tests. It is available to women of all ages in the District of Timiskaming. This clinic is run by a Registered Nurse.

Mindfulness Meditation for Individuals (Group Program)
This 6 week group program is intended to provide people with an alternative way to cope with chronic pain. Each group session has an educational component as well as a skill teaching component. 

Anger Solutions (Group Program)
Anger Solutions is a 10 week group program that is based on proven strategies that enable participants to effectively resolve anger and take control of their emotions. The program has a high success rate for all committed participants who complete the entire program.

Applicants must be adults of 18 years or older, a resident of Ontario, and a verified physical or mental disability that is expected to last a year or more. Disability must make it difficult for applicant to care for themselves, go into the community, or to work.

Available every Tuesday from the beginning of Septemeber to th end of June. No fees or appointment needed. Some topics that people wish to discuss include couple and family relationships, domestic violence, sexual assault, alcohol or substance abuse regarding themselves or significant others, financial or debt challenges, among others.

Developmental Services Program

Children and families can access us directly by calling. Adults, who are 18 years old or over, who need our services can apply through Developmental Services Ontario. Please ask for an APSW.  There are no fees for these services.

To qualify for a disability benefit under the Canada Pension Plan (CPP), a disability must be both "severe" and "prolonged", and it must prevent you from being able to work at any job on a regular basis.

• Severe means that you have a mental or physical disability that regularly stops you from doing any type of substantially gainful work.
• Prolonged means that your disability is long-term and of indefinite duration or is likely to result in death.

• To help you complete your application, read the General Information and Guide, which is part of the application kit. This guide includes steps for completing your application, changes that may affect your benefits, a mailing checklist and other useful information about disability benefits.
• If you are a parent or guardian, you could request the child-rearing provision (form is in the application kit) and the children's benefit (a section of the main application form).
• If you are unable to fill out the forms, a family member or a friend can help you. Make sure that you sign where necessary.

Applying:

• You must apply in writing. Print out the main application form (ISP 1151) and the consent forms, include certified true copies of the required documentation, and any other necessary forms from the application kit for CPP disability benefits, and mail them to us.
