Ontario Disability Support Program (ODSP)

Offers financial support to help cover costs of food, shelter, transportation and clothing while supplying health benefits for clients and their families. Also provides assistance in finding employment, which may include access to education, job counselling and job training. Applications go through DTSSAB or MCSS offices. 

Applicants must be adults of 18 years or older, a resident of Ontario, and a verified physical or mental disability that is expected to last a year or more. Disability must make it difficult for applicant to care for themselves, go into the community, or to work.

Apply and verify eligibility in-person, via phone or by the online application form. Once a patient has been verified as eligible, they will receive a package which includes the disability assessment form to complete with a health care professional.
