Call to be assigned a Care Coordinator. They will call you to provide an assessment, meet with you in your home environment and help determine what services and supports you may need. For people of all ages who are facing the challenges of age, illness, injury, or disability.

Referral Phone service and Website directory provides access to healthcare information and services in Ontario.

Services available to eligible patients may include:

• personal support
• nursing
• nurse practitioner
• physiotherapy and occupational therapy
• social work
• nutrition and food counselling (dietitians)
• access to long-term care homes
• access to supported living (adult day programs, supportive housing, retirement homes and assisted living)
• health support in the school
• medical supplies and equipment
• speech and language therapy
• referral to other community supports
