Temiskaming Hospital and Timiskaming Hospice Palliative Care are dedicated to providing the collaborative delivery of palliative care to those we serve. We strive to help individuals and families enhance the continuity and quality of life as they transition to the end-of-life.
Our Focus is on providing individualized care for each patient and family members while being sensitive to issues that may arise with physical needs, disease management, psychological needs, social, spiritual and practical needs, and those needs surrounding loss, grief and end of life support.
Admission Criteria for the Hospice Bed
have a progressive life limiting illness;
have a Palliative Performance Score (PPS) of 40% or less; priority will be given to a lower PPS scoring;
live in the district of Timiskaming and surrounding area, or wish to return to this area to be with familyat end of life;
no longer wish to receive active disease modifying treatment;
“Do Not Resuscitate (DNR)” has been established through the physician’s order, and DNRC form;
have care needs that exceed the supports available from the family (primarycaregivers) &/or care needs that exceed the supports available in the community or the individual does not wish to die at home;
have consented to admission to hospital / hospice care and will be accompanied by family members as required;
have a life expectancy which is anticipated to be less than 3 months; and
have been assessed by a Physician or Nurse Practitioner (N.P.) within the last two (2) weeks.
