ASRS (Adult ADHD Self-Report Scale Symptom Checklist) 
Adult ADHD Self-Report Scale (ASRS-v1.1) Symptom Checklist. The Symptom Checklist is an instrument consisting of the eighteen DSM-IV-TR criteria. Six of the eighteen questions were found to be the most predictive of symptoms consistent with ADHD. These six questions are the basis for the ASRS v1.1 Screener and are also Part A of the Symptom Checklist. Part B of the Symptom Checklist contains the remaining twelve questions.
BDI-II Beck Depression Inventory Second Edition 
Beck Depression Inventory is a 21-question multiple-choice self-report inventory, one of the most widely used psychometric tests for measuring the severity of depression. It is intended to assess the severity of depression in psychiatrically diagnosed adults and adolescents 13 years of age and older. BDI-II items are rated on a 4-point scale ranging from 0 to 3 based on severity of each item. The maximum total score is 63. 0–13: minimal depression 14–19: mild depression 20–28: moderate depression 29–63: severe depression.
